Looks like they could have put some more gravel (stemming) in the holes - lots of energy being expelled out the top instead of radially.

When I was doing some exploring in Tennessee I took a wrong turn down a road where they were blasting. They were about to blast so they wouldn't let me through or even turn around. It was pretty freaking awesome to see a 50 yard section of earth just swell upwards like a wave. Do you know what the small explosion in the gray material in the background could be? The Town of CUba has a Gated Comunity called Indian Hills or some thing like that it was built to be NICER than the surounding area it has a nice big lake and a comunity building it was the place to live 15 years ago today its just like everything else

Once a hole is drilled, explosives are packed in (tamped down in with a long rod) and capped off with loose gravel from the drilling process. The gravel is called stemming.
If you just lightly sprinkle the stemming in (and not tamp it) or not put enough down the hole, the drill hole will act like a shotgun barrel. The explosives will eject the stemming like a load of shot vertically up the hole and into the air. This wastes a portion of the energy of the explosives, sending the stemming up, instead of burden (rock) radially outwards.
Notice it was just a couple (far) holes that weren't packed as well as the close holes.
Not bad, but I just had to be an internet warrior and criticize it.
